Requirements
- Duly filled and signed application form.
- 1 recent white background photographs (size 35 mm * 45 mm).
- Original and photocopy of the Passport.
- Original and Photocopy (DEP, STP, REENTRY PERMIT) of CHILD.
- Application form dully filled with child thumbprint for a child below 15 years.
